The treatment card, or patient chart, is an essential tool in a medical facility and a very important document in working with the patient. It is used to record the diagnosis and treatment plan. Scheduled appointments are entered into it. The Treatment Card should always contain basic contact information such as name, surname, home address, telephone o e-mail address. In addition, information from the health history is included in the Card: a list of medications taken, past illnesses and any contraindications to treatments. A key part of the document is a record of all medical procedures carried out at the facility. The treatment card makes it possible to analyse the effects of the actions carried out. In the case of aesthetic medicine, the treatment card is additionally accompanied by before and after photos of the procedures performed. This allows the patient to analyse the results of the treatment in an excellent way. An important piece of information for the facility where the Patient Card has been created is the consents included in it. The Patient can decide whether he or she authorises someone to obtain medical information about him or her. Give consent for marketing communications from the facility.
