Treatment card

Treatment card, also known as a patient record, is a document containing key information about the patient, their state of health and the course of their diagnosis and treatment. It is used in healthcare facilities, including doctors’ surgeries aesthetic medicine.

The treatment record contains the patient’s basic details and information from their medical history, including details of any medication they are taking, past illnesses, allergies and any contraindications to procedures. It also contains information on consultations held, procedures carried out, recommendations and the planned course of treatment.

In the case of aesthetic medicine procedures, the documentation may also include photos taken before and after the procedure, which make it easier to assess changes occurring during treatment. The chart also records the required patient consent, including those relating to the conduct of specific procedures and other matters concerning the provision of services.

Keeping a treatment record helps to ensure that medical records are organised and makes it easier to monitor the progress and outcomes of the treatment provided.
